Hospital discharge care
The hospital discharge process can be a critical time for patients and their family. After illness patients may be suffering from temporary or permanent impairment and may need assistance not previously required (including physical adaptations to their home and support services) in order to resume living at home.
At Angel Carers we understand that hospital discharge can be a point of crisis and needs to be handled with the full involvement of patients and professional carers.
We are here to offer our advice and help for a speedy discharge and ongoing care.
Speeding discharge and avoiding overstays
In April 2024 NHS statistics showed that in the Southwest 1 in 5 beds was occupied by patients fit to be discharged. And these delayed discharges are largely of older people and due to lack of care availability in the community.
If you don’t want your loved one to be caught in this trap, with the health risks an extended stay could have, Angel Carers can help. We provide certified care in the comfort of your own home so patients can be discharged swiftly – with an approved discharge plan and care package.
